Synopsis
The goal of this presentation will be to provide an overview of the Collaborative Care model and the process used for implementation. The panellists will discuss the importance of change management support, and will identify the approaches needed to support nursing practice and collaborative practice. Specific implementation examples for in-patient care units will be given by our panel of experts in this informative webinar.

Jocelyn Vine
Jocelyn Vine, RN, BN, MHS, CHE, is the Vice President of Patient Care & Chief Nurse Executive at the IWK Health Centre, the tertiary pediatric and women’s centre for the Maritimes, and has worked at the IWK for the past 15 years. Jocelyn has worked in various health care settings, including the private sector. She has a strong interest in active engagement of patients and families as health team members, quality and patient safety, inter-professional practice, and evidence-based organizational decision-making.

Heather Simmons, RN, MSN, completed both her undergraduate and graduate degrees in Nursing at Dalhousie University. Heather has worked at the IWK Health Centre for over 28 years in a wide variety of roles including Manager of Interprofessional Practice, Project Lead for Models of Care, Professional Development Consultant, Health Service Manager, Support Coordinator, Clinical Leader and staff nurse.
