Objective

 The Family-Managed Home Care (FMHC) is an alternative method for home care service delivery that is intended to:

  1. Give patients or Substitute Decision Makers (SDM) greater choice over who provides services in their homes and when services are provided;
  2. Enable more flexible service delivery arrangements;
  3. Reduce service provider turnover; and
  4. Support long-term therapeutic relationships between patients and their chosen service providers.
